  If you have any insurance that requires a special referral from your personal physician please call their office to assure that the referral has been done .

If your insurance contract requires for you to pay a COPAY per office visit , please talk to me for  alternative payment options . Also do not forget to bring your insurance card(s) so that we can verify the address where to send your claims .

If you are not sure about your insurance coverage , please call your insurance carrier ( the # should be located on the front or back of your card ) and ask them for a description of your coverage .

WORKMAN's Compensation cases :   We must have a letter of authorization from your work compensation insurance company before we can see you .
